Demographic Information  

 

When you’re filling out a ‘Client Profile’ for a speech therapy patient, one of the first thing you’ll do is add their first and last name, their gender, and their date of birth. This is pretty basic, but it deserves to be mentioned nonetheless.

 

It’s really important to keep track of your patients’ date of birth in a speech therapy clinic because you might treat both adults and pediatric patients. For reporting purposes, you should be able to divide your patients by young and old.

Guardian Information

 

Many of our users are from pediatric SLP clinics. And quite often, the young patients who frequent their facility receive insurance coverage from one of their patents’ policies. In these cases, it makes it’s crucial to have their contact information and policy information handy.

 

Practice Perfect EMR includes a special tab in the ‘Client Profile’ especially for your pediatric patients’ guardians. This enables other key information, like policy coverage, to be easily completed.

 

Physician Information

 

As a speech language pathologist, some of your patients may be referred to your clinic from family doctors and general practitioners. And depending on the insurance carrier or regional healthcare provider who you’re submitting claims to, you may be required to include the ‘referring physician’ on your invoice submissions. Knowing this, Practice Perfect features a unique section in the software where you can create listings for all of your referring physicians.

 

Diagnosis

 

Patients coming to your SLP clinic have likely been diagnosed with a condition that necessitates treatment. Or maybe they’ve yet to be diagnosed at all and it’s your job to do so. In any case, it helps when there’s a specific place in the ‘client profile’ to record this information.
